Basic referral information:
- Client information: name, date of birth, social security number, ethnicity
- Medicaid status if known
- Access Center (Intake)
- Client’s current location
- Legal Guardian information: name, address, phone
- Juvenile Probation and Parole Officer, Guardian Ad Litem, Attorney contact information – as applicable
- Reason(s) for referral
- Level of care/services being requested (or may be determined by assessment)

24-Hour Care:
Residential Treatment Care
Treatment Foster Care
Transitional Living Services
Outpatient Services:
Day Treatment (3-4 hrs/day Mon-Fri)
BMS – Behavior Mgt. Services
Case Management
Outpatient Therapy

Who do we serve?
We help young people cope with the problems and pressures of growing up in a fast paced, high-pressure world…problems such as difficulties at home or at school and drug and alcohol use.
Who can make referrals?
Parents, school personnel, therapists, mental health workers, social workers, probation officers, anyone who has an interest in the client.
What kinds of insurance does Hogares take?
Private insurance
Medicaid:
NM Medicaid (exempt)
Cimarron Salud
Lovelace Salud
Presbyterian Salud
Non-insured- can go on sliding scale fee
*If you are unsure of what kind of insurance or Medicaid, the Access Center is able to help you find the resources to help you.
What educational opportunities are available to Hogares clients?
- Hogares does not have an on-site school
- Hogares clients attend schools throughout the community including APS schools
- Charter schools are available for students with specific needs or interests.
- GED preparation assistance is available to most of our clients
- Attendance at colleges or other learning centers is facilitated
